Our approach to complex support

How we assess, plan and deliver support for people whose needs may not have been met elsewhere, shown through a case example.

Case example

John’s situation

John is a young man with a learning disability who is autistic. He is moving from a care home to his own tenancy with support. He has limited verbal communication.

When his sensory and communication needs are not met, John becomes very anxious. This has led to damage to his environment, serious injuries and hospital admissions, causing further distress and trauma. He does not like to share his living space, including with support staff, and has a history of fire setting.

John benefits from clear boundaries and consistent approaches. He needs support to self-regulate, engage in meaningful activities and visit his family, which he enjoys.

Seeing the person, not the risk

Our teams are trained in trauma-informed care. We understand John’s behaviour as communication: when his needs are not met, anxiety follows. Our job is to understand what he is telling us and respond to that.

How we support John

Structure, safety and choice

A calm, predictable home

A settled environment, clear routines and visual schedules help John feel safe and in control, reducing his anxiety.

Safety planned in from the start

Risks are planned for before John moves in, so he can safely have more independence, not less.

A consistent team

A core team who know John well work alongside his family and the professionals involved in his care.

Communication that works for him

Pictures, symbols, Makaton and other methods, with speech and language therapy and advocacy where needed.

Positive Behaviour Support

Our in-house PBS specialists build practical, everyday plans around John’s goals, using non-restrictive approaches.

Choice and control

John is supported to make his own decisions, with the least restrictive option always chosen.

Referral and assessment

Clear, timely decisions

People are more than the risks and labels in their paperwork. Referrals go to our Enablement Panel, with operational and PBS input, and clinical input where appropriate.
