Who we are
Horizon Care is a specialist care and support provider for adults with learning disabilities, autism, mental health needs, brain injuries, physical disabilities and complex support needs, including people with substance misuse and co-occurring needs or forensic and criminal justice backgrounds.
Through supported living, outreach, residential care, short breaks and holiday stays, we provide personalised support shaped around each person’s needs, choices and ambitions.
We work closely with each individual, their family and the professionals around them to promote stability, independence and a meaningful life in their community.
